This form documents discrimination and harassment based on a protected class, sexual misconduct, and child abuse/neglect. For a list of protected classes, please see the university’s Non-Discrimination, Harassment, and Sexual Misconduct Policy.
Reporters can utilize this form to file a report on their own behalf or on behalf of an impacted party.
To fulfill a duty to report under the Non-Discrimination, Harassment & Sexual Misconduct Policy or the Youth Activities and Programs Policy, you must include your name and all known details about the incident, including name(s), date, location, and description.
An incident report does not, on its own, initiate an investigation. An investigation will only begin after a signed complaint is submitted to the Office of Institutional Equity (OIE), either by the complainant or by an OIE director. A Civil Rights Intake Coordinator will reach out with resources and options, by phone, email, or both.
